Coordination of care is an interdisciplinary approach to healthcare that includes that patient in the planning and interventions in a cost-effective manner (Lubkins, )  This care model can be particularly helpful in managing chronic illness by helping keep more people independent rather than relying on an all-inclusive care setting such as a nursing home. With a disproportionate amount of healthcare costs contributed to chronic illness it is important that we coordinate care for these patients (Friedman, Jiang & Elixhauser, 2008).  Coordination of care also reduces cost because it reduces the chance of ER visits and hospital re-admissions.
