My sample patient: 77-year old patient with a history of HTN, DM2, Hyperlipidemia, seasonal allergies, Neuropathy, anxiety, constipation, and Dementia. She still is noncompliant and gets up at random despite repeated warnings from staff. She is right outside the nursing station and would not have to wait for help if she would just ask them. As a result of this, she has had multiple falls in the past and most recent was yesterday. She has a hematoma about the size of a nickel from the fall and neuro checks were done. Pain rated 5/10. Tramadol helps. She remains stable. Current medications are ALPRAZOLAM TAB 0.5MG 1 tablet by mouth three times per day as needed. AMLODIPINE TAB 10MG 1 tablet by mouth once daily. ASPIRIN TAB 81MG EC 1 tablet by mouth once daily. BUSPIRONE TAB 15MG 1 tablet by mouth at bedtime. CLONIDINE HCL 0.52 mg 1 tablet by mouth once daily. LIPITOR TAB 10MG 1 tablet by mouth at bedtime. METFORMIN TAB 500MG 1 tablet by mouth twice daily. METOPROLOL SUCCINATE ER 25mg 1 tablet by mouth once daily.
