5 Wishes is a living will that was created by the American Bar Association’s Commission on Law and Aging. It is not a right to die, or death with dignity tool. Rather, it is a document that allows individuals to choose who they would want to make health care decisions for them if they were unable to make them for themselves, and it asks questions that prompts people to consider think how they would want to be treated in the event they become become seriously ill.

This week read through this document and consider what your wishes would be, how did you respond to the questions listed? Perhaps this will even lead to a conversation with your loved ones. We would like for you to write a 250-500 word response to your experience reviewing this tool. Questions you may want to respond to include: Was this the first time you had ever considered these things? Did your loved ones already know your wishes? Did you know theirs? Do you think it is valuable for a young, healthy adult to discuss end of life wishes? Were your wishes influenced by the information in the module or the podcast? What was the greatest influence in your decision making? 

Clearly there are no right or wrong answers on this assignment, this will be an expression of your opinion. However responses that are thoughtful, well written, meet the length requirements and are posted on time will receive full credit.  No outside references are required.
